Prairie Community Services, Inc. (PCS) provides services foradults who have physical, intellectual, medical, mental health and behavioral support needs. Each program serves two to four individuals, in a shared comfortable home. Each individual is supported by their identified needs and staff is present to assist them as needed 24 hours a day. All the programs are licensed by the Department of Human Services, following Minnesota 245D statutes and as Adult Foster Care Homes.

At PCS, we offer a comprehensive list of services to help individuals across Minnesota overcome challenges and live fulfilling lives. Our services are tailored to meet the needs of the individuals we serve, with a focus on personal growth and independence.

  • Adult Foster Care (AFC): Community residential programs for adults 18 years of age or older with intellectual disabilities, mental illnesses, traumatic brain injuries, and related conditions. Providing personalized care for small groups of two to four, with staffing tailored to individual needs.
  • Intensive Residential Treatment Services (IRTS), Located in Marshall and Duluth: Short-term care (up to 90 days) for adults with serious mental illness, focused on supporting the transition back into the community.
  • Community-Based Services: Provides a range of licensed support services, including 1:1 home support and training, respite care, crisis respite, 24-hour emergency assistance, companion services, homemaker services, community living support, and employment services.
