At St. Francis Health services, we believe Behavioral and Residential Services should empower individuals with disabilities to achieve their life’s fullest potential. Every resident should be treated with dignity and respect, while receiving personalized services in an inclusive and supportive environment.

For over 20 years, our service partner, Prairie Community Services, Inc. (PCS), has raised the standard in care, housing, and licensed services for individuals with developmental and delayed disabilities, mental illness, traumatic brain injuries, and other health-related needs throughout Minnesota. Through PCS, we offer a variety of care options, including intermediate care facilities, adult foster care, children’s foster care, and intensive residential treatment.

Adult Foster Care (AFC) Located in Fergus Falls: Community residential programs for individuals ages 18 years of age or older with intellectual disabilities, mental illnesses, traumatic brain injuries, and related conditions. Serving two to four individuals with staffing based on individual needs.

Intermediate Care Facility in Fergus Falls: A licensed home serving up to five individuals 18+ with intellectual disabilities prior to age 22 under Department of Health standards.

Intensive Residential Treatment Services (IRTS), located in Marshall and Duluth: Short-term (up to 90 days) care for adults with serious mental illness, focusing on transitioning back into the community services billed through insurance.

Community-Based Services: Offers a variety of licensed support including home support services with 1:1 training; respite care, crisis respite, 24-hour emergency assistance, companion services, homemaker services, community living support, and employment services.

Personalized service and supports for individuals with disabilities

St. Francis is dedicated to empowering individuals with disabilities by providing the tailored support and person-centered care they need to thrive. Through PCS, we offer a range of programs designed to promote independence and improve quality of life, including:

  • Trained full- and part-time staff available 24 hours a day
  • Care and programming for individual needs supervised by a QMRP (Qualified Mental Retardation Professional), mental health professionals, a licensed psychologist and psychiatrist
  • Registered nurses, dietitians and other healthcare professionals address medical and nutritional needs
  • Participation in life skills training, day placement, day treatment, community integration activities, and vocational readiness programs
