In-Home Services

Our in-home services provide the right amount of assistance so individuals can remain living independently. Habilitation services provided to a person and his/her family, including extended family members who are not providing licensed foster care, in the family’s home and/or in the community to enable the person to remain in or return to the home. This includes training of the person and family members to increase their capabilities to care for and maintain the person in their home.

PERSONAL CARE ASSISTANTS

Services that provide assistance and support for persons with disabilities, living independently in the community. This includes the elderly and others with special health care needs. PCA services are provided in the recipient’s home or in the community when normal life activities take him/her outside the home.

SEMI-INDEPENDENT LIVING SERVICES

Services are available on an à la carte basis to individuals who live independently. Our in-home services provide the right amount of assistance so individuals can remain living independently.

Community Support Programs

Services consist of mental health and substance abuse rehabilitative services and supports necessary to assist the individual in achieving and maintaining the highest degree of independent functioning. The service utilizes a team approach to provide intensive, rehabilitative community support, crisis intervention, group and individual psycho-education, and skill building for activities of daily living.

Independent Living Services

Services that develop, maintain and improve the community-living skills of a person.

Respite care services

Short-term care services provided due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.

Supportive Employment

Paid employment for participants for whom competitive employment at or above the minimum wage is unlikely without supports, and who, because of their disabilities, need intensive ongoing support to perform in a community work setting. Supported employment is conducted in a variety of community work settings in which people without disabilities are employed.

Supported Living Services

Services provided to a person who cannot live in his or her home without such services or who need outside support to remain in his or her home. Habilitation services are provided in the person’s residence and in the community, and should be directed toward increasing and maintaining the person’s physical, intellectual, emotional and social functioning.
