At Prairie Community Services (PCS), we believe every person deserves to live with dignity, purpose, and independence. We provide residential and community-based services tailored to adults with physical, intellectual, medical, mental health, and behavioral support needs throughout Minnesota.
Whether it’s a long-term home or short-term transitional support, our programs focus on individual strengths and needs—empowering people to live meaningful, independent lives.
All services are licensed by the Minnesota Department of Human Services, operating under the 245D statutes and as Adult Foster Care Homes.
Prairie Community Services’ Mission
At PCS, we proudly uphold the mission we share with our parent organization, St. Francis Health Services: to “express Christ’s love by providing care that values every human life.” This mission guides our daily work as we ensure each individual receives the personalized support and resources they need to live with dignity, purpose, and independence.

Behavioral and Residential Services
Adult Foster Care (AFC): For adults 18+ with intellectual disabilities, mental health diagnoses, traumatic brain injuries, or other related conditions. We provide 24-hour staffing in homes serving two to four people, with personalized support plans that foster independence and daily success.
Intensive Residential Treatment Services (IRTS) – Marshall & Duluth: Short-term, structured mental health treatment (up to 90 days) for adults experiencing serious mental illness. These programs support individuals in stabilizing and successfully transitioning back to community living.
